Preceptor

All Preceptors must register with the Board and identify the School(s) with which they have or will have an affiliation on his/her annual renewal application. Should a licensee become a Preceptor between renewal cycles, he/she must register by use of the following form with the Board within 30 days. There is no fee to register with the Board. The Board has the right to terminate a Preceptor or Preceptor site for reasons such as a disciplinary issues or other issues relative to the public welfare.
